ODE Forms
The Office for Exceptional Children (Ohio Department of Education) has revised the state’s Evaluation Team Report (ETR) and Individualized Education Program (IEP) forms. The new forms now have tool tips throughout, providing guidance while educators complete them. The new forms are available online for use. They are required for the 2018-2019 school year.

A Guide to Parent Rights in Special Education
A special education procedural safeguards notice, A Guide to Parent Rights in Special Education, is available for district and parent use. This publication replaces the earlier procedural safeguards notice, Whose IDEA Is This? It is available in several languages.
